About HCBS and Habilitation + Other Residential Services

DAC, Inc.’s HCBS (Home and Community Based Services) and Habilitation programs currently serve approximately 220 adults and children. To qualify for these programs, individuals must have a diagnosed brain injury, intellectual disability or chronic mental illness. Many of the individuals served are adults who receive 24-hour support in DAC-owned homes. In the HCBS and Habilitation programs, each person has an individualized plan created to assist that person in becoming as independent as possible. In the Habilitation program there is a strong emphasis on supports that assist with the acquisition, retention, or improvement in skills related to living in the community.

The HCBS and Habilitation programs employ approximately 250 direct support staff and 18 supervisory staff in Jackson, Clinton, Jones, Scott and Dubuque counties.
