Prairie House, Maquoketa

Prairie House is a 12-bed ICF/ID (Intermediate Care Facility for Intellectually Disabled) located in Maquoketa , Iowa . The individuals served all have a primary diagnosis of mental retardation, and may require a high level of support. Beyond that, the individuals have a variety of secondary diagnoses including autism, cerebral palsy, communication deficit, behavioral disorders, as well as a wide variety of medical diagnoses. Several of the individuals require the use of a wheelchair for mobility and positioning. Many of the individuals served at Prairie House are non-verbal.
Prairie House employs approximately 27 direct support staff and 5 supervisory staff. There is trained staff on duty 24 hours per day, seven days a week. Individuals served at Prairie House receive individualized program planning designed to enhance their abilities and gain independence in daily living skills.

Home and Community-Based Services, Habilitation and other Residential Services

DAC, Inc.'s HCBS (Home and Community Based Services) and Habilitation programs currently serve approximately 75 adults and children. To qualify for these programs, individuals must have a diagnosed brain injury, mental retardation or chronic mental illness. Many of the individuals served are adults who receive 24-hour support in DAC-owned homes. In the HCBS and Habilitation programs, each person has an individualized plan created to assist that person in becoming as independent as possible. In the Habilitation program there is a strong emphasis on supports that assist with the acquisition, retention, or improvement in skills related to living in the community.
The HCBS and Habilitation programs employ approximately 50 support staff and 6 supervisory staff in Jackson, Clinton, Jones, Scott and Dubuque counties.
